The Community Health Foundation strives to create a Connecticut where everyone can achieve optimal health, regardless of race, ethnicity, and socioeconomic status. Focused in four areas that include coverage and access, improved care delivery, advocacy and leadership, and linking care to the community, they work to ensure that the next generation will not face the same persistent racial and ethnic health disparities as their parents. This project allowed for the Connecticut Health Foundation to hire and train twenty Community Health Workers over a 6 month period and deploy them into communities of color that have been disproportionally impacted by COVID-19.
